Chronic disease resources

Independent Living Centres are located in every state and the ACT. Each centre displays a wide range of products and equipment to assist with daily living.

Everybody is welcome to use these centres’ services, including people with disabilities, older people and their carers. You can try out products and equipment at a centre before buying what you need from commercial suppliers. Please note that Independent Living Centres don’t sell things, they only display them. Most of the services are free to the public. Contact details in each state are provided on their website.

If you have a chronic disease you can be assessed for the community care (more information) programs of the Australian Government. These programs help you to maintain your independence while continuing to live in the community. For more information or to arrange an assessment, contact your nearest Commonwealth Carelink Centre (more information) on 1800 052 222.

Aged care homes cater for the needs of people with chronic diseases - each individual needs to apply to a home that will help them with their particular condition. In the case of dementia and palliative care (more information), there are homes that do have these services and others that do not.

Information on particular chronic diseases

There are information sources on most chronic conditions, including telephone information lines in some cases, or websites that provide details of information or support services.
