Community nursing and health centres
Community health centres aim to improve your health and well-being by:
- encouraging you to actively participate in your own health care
- working on your needs with other primary health care providers
- offering facilities for your local community groups
- encouraging active participation in the centre's activities, including involvement in management, fundraising and volunteer work
- promoting prevention of lifestyle-related diseases and conditions, and
- developing health care programs and activities to improve social and physical environments in the community.
Community health centres offer a range of services that can help you remain at home by supporting you and your carer. The types of services that may be available at your local health centre include:
- medical services
- physiotherapy to help maintain your flexibility, strength and movement
- podiatry for foot and lower limb problems
- nursing, including health advice, education, counselling and monitoring
- speech pathology to help you communicate more effectively should you have a speech disorder
- social workers to help you or your carers with problems related to finances, accommodation or socialisation, and
- tailored programs for your specific dietary needs.
Home nursing
A nurse may also visit you to help:
- restore health after an illness, and
- allow you to maintain the best level of independence.
They may also provide assistance with:
- personal hygiene
- medication
- injections, or
- changing dressings.
