Community nursing and health centres information for carers
Community health centres aim to improve the health and well-being of the person you care for by:
- encouraging them to actively participate in their own health care
- working on their needs with other primary health care providers
- offering facilities for local community groups
- encouraging active participation in the centre's activities, including involvement in management, fundraising and volunteer work
- promoting prevention of lifestyle-related diseases and conditions, and
- developing health care programs and activities to improve social and physical environments in the community.
- medical services
- physiotherapy to help maintain their flexibility, strength and movement
- podiatry for foot and lower limb problems
- nursing, including health advice, education, counselling and monitoring
- speech pathology to help them communicate more effectively should they have a speech disorder
- social workers to help both of you with problems related to finances, accommodation or socialisation, and
- tailored programs for their specific dietary needs.
Home nursing
A trained nurse may also visit them to help:- restore health after an illness, and
- allow they to maintain the best level of independence.
- personal hygiene
- medication
- injections, or
- changing dressings.
