Heart Foundation

The Heart Foundation is an independent Australia-wide, non-profit health organisation, funded almost entirely by donations from Australians. The Heart Foundation's purpose is to improve the heart health of Australians and to reduce disability and death from heart, stroke and blood vessel disease.

High-level care

High-level care is the care which is provided for people who have been assessed by an ACAT, and need almost complete assistance with most daily living activities. It includes accommodation services such as meals, laundry and room cleaning, as well as personal care. Medical needs are managed by nursing staff. High-level care was previously known as ‘Nursing Home' care. See also specified care and services.

Home and Community Care Act 1985

The Home and Community Care Act 1985 is the Commonwealth legislation that governs financial assistance for home and community care services, the planning of services, and for people who are to be assisted through the program.

Home and Community Care program

Home and Community Care (HACC) provides a comprehensive, coordinated and integrated range of basic maintenance and support services to help people maintain their independence at home and in the community. It is a joint Australian and state/territory government program.

Home modifications

Home modifications are changes or additions to a person's house or local environment that allow them to live more comfortably, safely, and securely at home for as long as possible.

Hospice

A hospice is a facility devoted to the care of people with a progressive life limiting illness, which is staffed by specifically trained doctors, nurses, social workers, physiotherapists, and volunteers. It offers total care for the patient, including physical, emotional and spiritual support, and care also for the family.

Hostel care

‘Hostel Care’ is now known as ‘low-level care’, and refers to accommodation services such as meals, laundry and room cleaning, as well as additional help with personal care, and nursing care if required. 'Hostel care' is low-level care provided in an aged care home.
