| Answer: | It is helpful if you provide the Care Manager with a copy of your Advanced Care Directive. This should be included in your care plan and discussed with you regularly to ensure that your wishes are up-to-date. You can also appoint an enduring power of attorney or enduring guardian who knows your wishes to make decisions about your health care if you are no longer able to do so.
